Valid Insurance Verification (with current dates) is required for all registration & renewals - bring in or email cards to tags@cherokee.org with the office location in the subject line.

Please complete, sign, and return form 56-01A or 56-01B to tag-office@cherokee.org (use "Tag Office: Address Change" on the subject line) or fax to 918-458-7664 or mail to Cherokee Nation Tax Commission P.O. Box 948 Tahlequah, OK 74465

Identification Required – Submit One of the following with a completed 56-01A or 56-01B form.

  1. A valid Oklahoma driver’s license
  2. A valid Oklahoma state issued ID
  3. Cherokee Nation citizenship photo ID
  4. Valid unexpired Military ID

Please include all vehicles, boats, & motors you have registered with us.

Proof of address of change from At Large jurisdiction to the Cherokee Nation Reservation must be provided.

  1. A valid Oklahoma driver’s license showing the citizen’s physical address within Cherokee Nation Reservation; or
  2. Current utility bill (Electric, Gas, Water, Trash/Recycling, Internet/Cable, or Home-Telephone) showing citizen’s physical address within Cherokee Nation Cherokee Nation Reservation; or
  3. Three (3) pieces of current mail dated within the last 30 days; showing the citizen’s physical address within Cherokee Nation Reservation.
    Specifically meaning - mail must be in an envelope with a postmark. Accepted mail includes the following; letters, personal correspondence, bills, and invoices only; or
  4. Cherokee Nation membership photo ID showing a physical address within Cherokee Nation Reservation; or
  5. Oklahoma State photo ID showing citizen’s physical address within Cherokee Nation Reservation.

Name Change: If name(s) has changed, "e.g." the vehicle registration does not match the signature on the change of address form, document(s) must be submitted. Marriage License, Divorce Decree, Legal Document for name change signed by a judge, etc.
